Based on the provisions of § 1829 paragraph 1 in connection with § 1818 of Act No. 89/2012 Coll., the Civil Code, I hereby exercise my statutory right and withdraw from the purchase contract concluded via the internet concerning the goods _______________________________________________________________________.

The buyer acknowledges that the seller is not obliged to refund the purchase price if the goods are not returned within 14 days from the delivery of the withdrawal notice or if the buyer does not prove that the goods were sent to the seller within the 14-day period.

The buyer also acknowledges that the costs necessary to return the goods to the seller shall be borne by the buyer as part of the withdrawal from the contract.
